If you do not have
Microsoft Word on your computer you can download
OpenOffice which includes a free Word processor
Once you click on the Download button you will be directed to the OpenOffice website. From there click on the link "I want to download OpenOffice.org" and install the software. |

Download the current Schedule Change form
|

Once you have the form saved to your computer you can
open the form by double clicking on the form.
Make sure you DO NOT
change the form layout by adding additional lines or
removing any information. Scheduling does not have
to accept the form if it has been changed. |

Make sure you fill out the form COMPLETLY. Your Name, Employee #, Base and Bass Bid # Use the Bid # from the Bid Packet and NOT the one from FLICA |

Make sure to check either Regular Line Holder, Move Up
line, or Reserve Line boxes |
| 6. |
You can only use the form for ONE
request at a time. Example, lets say you want to
pick up a trip from Open Time but you also want to Drop
a Trip into Open Time. You MUST e-mail 2
different forms. Make sure somewhere on
the form itself you number them 1 of 2 and 2 of 2 so
that scheduling knows in what order you want them
processed. |

Make sure to check the applicable boxes at the bottom of
the form I wave my ( ) Calendar Day Off ( ) 33 /7 Limitation ( ) Minimum Days Off |

Before you e-mail the form make sure to SAVE
the changes. If you are using OpenOffice or a
program other than Microsoft Word. MAKE
SURE to save the form as a WORD document.
If the program asks you to convert the document you want
to make sure to say NO and keep it in the WORD format. |

You are now ready to e-mail the form to Scheduling.
Bring up your e-mail program and in the To: box type
opsscheduling@spiritair.com |

In the Subject line type Schedule Change Request |
| 11. |
In the message of the e-mail you can include a message
to Scheduling about the request or just tell them that
the form is attached |

MAKE SURE to attach the document to your e-mail message.
This is done differently in each program so I can not
list those instructions here |

Once the document is attached you can SEND the message.
Tip: a good idea is to send a copy to your ra.spiritair.com e-mail address. In the CC: box of your e-mail type your Company e-mail address which is FL followed by your Employee ID # @spiritair.com (FL51234@spiritair.com) |
