DEPENDENT ELIGIBILITY
Ladies & gentlemen, this is a reminder to get your forms in to Human Resources for any dependents you now cover on your health/medical insurance.
All children under and over 19 on your health insurance, you must show proof of parenthood.
IF YOU DO NOT GET THE FORMS IN BY AUGUST 31, 2009 THEY WILL BE REMOVED AND HAVE NO COVERAGE.
